Search hints:
  • The search tool is sensitive to exact spelling based on the spelling received in our records.
  • If unsure of the spelling or ending of a word, use a % symbol as a wild card in place of the unknown letters or the word ending.
  • When “Query Too Large” or you want to limit the number of results, search with more words or unique words for the employer name or add a city, street name, or street number.
  • Refine list of search results by typing additional information in the “filter” space found in upper right corner on the results page.
  • Sort lists by clicking on column headings.
  • “Claim Processing Information” displays an address when the insurer has only one claim processing location. If there is no address, contact the insurer or employer for the information.
  • The 'Previous Search' button will return you to the search screen, with your previous information filled in.
  • The 'Return To List' button will take you back to the first list area you reached after your search.
  • The 'New Search' button will take you back to the search screen with all information removed.
  • The browser's back button may not work on all browsers.
